Patient details: First and last name and date of birth
Issue date of the prescription
Details of the issuing doctor: first and last name, contact details or address of the practice (indicating the country), signature (handwritten or digital).
Information on the prescribed drug: its generic name (safer than the brand name, which may vary from country to country), dosage form (tablet, solution, etc.), dosage quantity

All prescriptions from abroad are treated as private prescriptions, meaning you pay the German market price for the drug.
